真实写回入口（gated / default blocked）
Real writeback is gated. POST /api/docops/changes/[id]/drive-writeback-real 默认 blocked；只有硬条件全部通过并收到最终执行触发时才会执行一次。
真实执行二次确认（disabled 壳）
真实执行二次确认尚未启用。必须完成 readiness 重新评估为 A、并获得人类 explicit 批准后，才允许启用。
